The HCPH outreach team is a diverse group of community Health Workers (CHWs) with a mission to improve the health and wellbeing of individuals in Harris County.

Connection to State and Community resources and services based on their individual needs. Food assistance-SNAP, WIC, and others. Financial assistance-TANF cash help, and others. Mental Health services. Immigration assistance. Maternal Mental Health resources. Health Texas Women-helps to pay for health exams, screenings, and birth control. Health Care- Medicaid, Medicare, CHIP, and others.
